Right to Access: You have the right to inspect and obtain a copy of your PHI in our records, subject to limited exceptions.
Right to Amend: You have the right to request an amendment to your PHI if you believe it is inaccurate or incomplete.
Right to an Accounting of Disclosures: You have the right to receive a list of certain disclosures we have made of your PHI.
Right to Request Restrictions: You have the right to request restrictions on certain uses and disclosures of your PHI.
Right to Confidential Communications: You have the right to request that we communicate with you about your health information in a certain way or at a certain location.
Right to a Paper Copy of this Notice: You have the right to receive a paper copy of this Notice upon request.
Right to Notification of Breach: You have the right to be notified in the event of a breach of your unsecured PHI.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with this Practice or with the Secretary of the U.S. Department of Health and Human Services.

We reserve the right to change this Notice at any time. We reserve the right to make the revised or changed Notice effective for PHI we already have about you as well as any information we receive in the future.

This Practice will occasionally update this Privacy Statement to reflect company and customer feedback. The effective date at the top of this document will be updated accordingly.
